When to use this template
This template is for asking a school or university for a leave of absence from your studies. It gives the reason and the date the leave should start.
What the letter contains
- Message gives the reason for the leave of absence, opening the sentence after “Due to”.
- Date is the day the leave of absence should start, and it is repeated above your name.
- No end date or length is given, so the letter does not say when you would return.
- Recipient is the school, faculty or student office that handles leave of absence.
- Your full name identifies the student; an ID number can be added on its own line.
- An address entered in the form goes under your name, below the date.
