When to use this template
This template is for asking a clinic or practice to move an appointment. It names the date of the appointment you have and the reason you need to change it, and asks for it to be rescheduled.
What the letter contains
- Date is the day of your current appointment, after “my appointment on”; it is also printed above your name.
- Message gives the reason for rescheduling, after “due to”.
- The letter asks for the appointment to be rescheduled but does not suggest a new time.
- It does not cancel the appointment outright; it asks to move it.
- Recipient is the clinic, hospital or practice.
- Your full name identifies the patient; an ID number line and an address are added under it only if you enter them.
